Benefit Applications

The Sedro-Woolley School District will no longer distribute and process Free and Reduced-Price Meal Applications.  To ensure that the other educational services are still available, families who previously qualified for Free and Reduced-price meals via application should complete the Benefit Application.  This application allows the district to track income data for families that is used to determine funding levels for a variety of educational programs offered throughout the district. 

Completed applications can be returned to the school office or Food Service office at 780 Cook Road, Sedro-Woolley, WA 98284.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can also be obtained from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
